Mapleton School District is offering up to $150 reimbursement to qualifying families for internet services held during the months of March, April, May, June, September, October, November, December 2020 and January, February, March 2021.
To receive the reimbursement, submit this form with a copy of receipts/invoices/bills totaling up to $150 from the listed months. For example if you spent $50 a month then you could attach 3 months worth of bills to show the $150. If you spent $20 a month you could attach 8 months to show $150. If you only had $80 worth of bills you would be reimbursed that amount. To be a qualifying family, you must not have received internet from the district through the use of a hotspot and you will need to have bills detailing internet service for one or more of the listed months.
